Summary
The Social Worker will be assigned to the Tomah or Community Based Outpatient Clinic (CBOC) Patient Aligned Care Teams (PACT). The Social Worker must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans. Social Work responsibilities include the assessment of social drivers of health often at a time when the Veteran is experiencing crisis and include the provision of short-term, solution focused counseling with Veterans and/or their families.
Duties
Major Duties: Clinical Functions: Demonstrate a high level of skill and expertise to establish and maintain an effective therapeutic relationship with Veterans and/or their families, caregivers, or survivors. Works independently with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems. Applies advanced interpersonal communication, evidence-based knowledge, and clinical expertise to guide the interdisciplinary team in the appropriate use of clinical services and to enhance the continuity of care and process improvement. Demonstrate an ability to complete thorough assessments to determine the psychosocial functioning and needs of Veterans and/or their families, caregivers, or survivors. Actively participates as a member of the interdisciplinary treatment team in the development and implementation of treatment goals and interventions as well as collaborate with Veterans and/or their families, caregivers, or survivors. Utilizes accurate data, relevant research, literature, and evidence-based guidelines to maintain and improve processes and Veteran outcomes. Administrative Functions: Attend all appropriate staff meetings (i.e. team meetings, social work staff meetings, social work supervision meetings, committee meetings, workgroups, etc., as appropriate) and perform other duties as required. Collaborate in performance improvement processes and comply with performance measures as required by the VA for their specific population and/or program. Participate in activities that obtain feedback from Veterans and work to enhance services as needed. Serve on committees and work groups at the facility as deemed appropriate by the supervisor and Chief of Social Work. Keep their supervisor apprised of concerns observed and recommends solutions in the incumbent's area of responsibility. Work Schedule: Monday-Friday; 8:00am to 4:30pm Recruitment/Relocation Incentives (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework: Not Available Virtual: This is not a virtual position. Requirements
