The Registered Nurse - Community Living Center Admission Coordinator provides evidence-based nursing care and coordinates CLC admissions, screening, and placement. Duties

CTVHCS is in the process of making a cultural change, shifting from a disease-focused approach to a health and wellbeing approach, in order to implement the Whole Health System of Care. This is a VA-wide initiative to provide a comprehensive and systematic approach to improve healthcare delivery at all stages of Veteran care. This entails moving from an episodic, "siloed" approach to an interdisciplinary, holistic approach, emphasizing self-care in the larger context of well-being. All staff in this facility will be part of this process, shifting the focus from the question, "What's the matter with you?" to the question, "What matters to you, and how can we help you live your best life?" This strategy also involves integrating a full range of conventional approaches with Complementary and Integrative Health (CIH) approaches in order to optimize lifelong health and well-being, as defined by the Veteran, rather than narrowly focusing on disease management. Major duties include, but are not limited to: Assesses, plans, implements, and evaluates nursing care using evidence-based and age-specific practices. Coordinates all aspects of CLC admissions, including screening, placement decisions, referral review, and communication of outcomes. Collaborates with interdisciplinary teams to identify Veterans appropriate for CLC programs and ensure smooth transitions into the CLC setting. Provides patient education, supports self-management, and promotes continuity of care across the continuum. Administers medications and treatments in accordance with established policies and clinical guidelines. Monitors bed availability and serves as the point of contact for admission-related decisions and referral workflow. Partners with VA and community agencies to address Veterans' needs and facilitate timely referral processes. Collects and maintains data to support program improvement, placement accuracy, and quality care outcomes. Participates in resolving complex administrative or clinical issues related to admissions or resident placement. Contributes to a safe, collaborative care environment through effective communication, teamwork, and support of staff development. Requirements

Education
