Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Obtains vital signs, assists with basic treatments, and reports observations to the RN case manager. GS-4 Responsible and accountable for individual nursing practices. Performs an expanded range of nursing procedures in the home setting for patients with moderately complex chronic conditions. Exercises limited independent judgement in adapting care to the home environment, monitors patient and caregiver responses, and communicates findings to the RN case manager. GS-5 Responsible and accountable for nursing practice in the home setting. Conducts home visits, implements the nursing care plan, administers medications and treatments, educates patients and caregivers, and recognizes and reports changes in condition that may require case manager or provider intervention. GS-6 (Full Performance) Responsible and accountable for individual nursing practice. Technically proficient in initiating, performing , and completing assigned duties in providing care to variable patient populations. Knowledge and ability to appropriately carry out assigned patient care based on the patient's conditions. Use judgement in selecting the appropriate order and sequence of procedures and treatments. Accurately recognize, report and record relevant patient information. Exercise high degree of independent judgement in the home setting in managing patients with complex, multiple, or unstable chronic conditions who require specialized monitoring and intervention. Performs advanced or technically difficult procedures within scope of practice. Identifies and addresses barriers to care unique to the home environment. Serves as a resource to other LPN/LVN staff on complex home-care cases. Operates with considerable autonomy between team check-ins, consistent with the independent nature of home-based care delivery. Requirements

You must be a U.S. Citizen to apply for this job.
