As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Schedule: Monday- Friday 8:00am-4:30pm with on-call coverage
Position Summary
The APRN exemplifies and promotes advanced nursing practice. Provides direct participant care and evaluates participant outcomes.  Sets and evaluates practice standards.  Provides consultation to nurses.  Maintains own professional knowledge and skills as an expert and role model in the PACE Center’s advanced nursing practice.  Works in close collaboration with Bluegrass Care Navigator’s Chief Medical Officer and the PACE Medical Director as well as community referral sources and medical specialists to establish the individualized participant centered plan of care (POC).  Assists in providing a plan of care for the participant/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, federal and state guidelines, and organizational policies and procedures, incorporating participant and caregiver input and requests as deemed appropriate.
Providers of Bluegrass PACE care deliver the full range of ambulatory primary care services, including diagnosis and treatment of acute and chronic illness in geriatric age groups, preventative care, minor surgical procedures, and occasional medical emergencies. The providers are responsible for seeing their scheduled participants as well as walk-ins and add-ons and provide coverage during the operating hours of the Center. Provide telephone consultation as needed, perform administrative tasks associated with the provision of medical care, participate in quality improvement activities and provide community service. Medical providers are expected to manage all participants and conditions within the scope of their training and approved privileges.
Position Qualifications
- Graduate of an accredited master’s degree nursing program.
- Certified as an Advanced Practice Registered Nurse (APRN) as a Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP).
- Currently licensed as an APRN in good standing with the applicable State Board of Nursing.
- Demonstrates in-depth, current knowledge in area of specialty and the ability to translate professional and organizational goals into effective clinical practice.
- Minimum of three (3) years of general nursing experience required; experience in palliative care, home health, long-term care, urgent care, or primary care preferred. I.V. skills preferred.
- Minimum of one (1) year of experience working with the frail and elderly population required.
- Demonstrates strong clinical nursing skills with the ability to manage family emotional stress and respect individual lifestyles and cultural differences.
- Committed to evidence-based practice and the application of research to clinical care.
- Demonstrates effective resource organization and time management skills.
- Exhibits strong interpersonal, communication, collaboration, and conflict-resolution skills.
- Knowledgeable of healthcare systems and organizational change processes.
- Ability to manage multiple priorities effectively and cope with the emotional demands of the role.
- Complies with accepted professional standards, ethical principles, and regulatory requirements.
- Demonstrates understanding of the organization’s mission, philosophy, and the PACE care delivery model.
- Maintains current CPR certification.
- Provides satisfactory professional references from nursing education, previous or current employers, and/or professional peers.
- Demonstrates excellent observation skills, sound clinical judgment, and strong written and electronic documentation abilities in a timely manner.
- Self-directed with the ability to work independently while functioning effectively within an interdisciplinary team.
- Flexible, cooperative, and able to adapt to changing priorities and role responsibilities.
- Must be a licensed driver
