Humana Gold Plus Integrated is seeking a Field Care Manager Registered Nurse (RN) to assess and evaluate member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members and families toward resources appropriate for the care and wellbeing of members. The Field Care Manager Nurse employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.

IdentifiesĀ and resolves barriers that hinder effective care.Ā Ā
EnsuresĀ patient is progressingĀ towards desired outcomes by continuouslyĀ monitoringĀ patient care through assessments and/or evaluations.Ā
CreatesĀ plansĀ ofĀ care.Ā
Communicates with internal and external stakeholders.Ā
Understands department, segment, and organizational strategy and operatingĀ objectives, including their linkages to related areas.Ā
Makes decisionsĀ regardingĀ own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.Ā
Follows establishedĀ guidelines/procedures.Ā
VisitsĀ Medicaid members in their homes,Ā SupportiveĀ Living Facilities, and/or Long-Term Care Facilities and other care settings ā 75-90% local travel (see Additional Information section).Ā
Upon hire, this role will be assigned to support either MLTSS or LTSS based on business needs.

Required QualificationsĀ
MustĀ resideĀ inĀ theĀ state of Illinois.
Registered Nurse in the state of Illinois without disciplinary action.Ā
2+ years ofĀ experience ofĀ in-home case/care management.Ā
Experience working with the adult population.Ā
Knowledge of community health and social service agencies andĀ additionalĀ community resources.Ā
Exceptional communication and interpersonal skills with the ability to quickly build rapport.Ā
Ability to work with minimal supervision within the role and scope.Ā
Ability to use a variety of electronic informationĀ applications/softwareĀ programs including electronic medical records.Ā
Intermediate toĀ Advanced computer skills and experience with Microsoft Word, Outlook, and Excel.Ā
Excellent keyboard and web navigation skills.Ā

Preferred QualificationsĀ
Bachelor of Science in Nursing (BSN).Ā
3+ years of in-home assessment and care coordination experience.Ā
Experience with Medicare/Medicaid members.Ā
Experience with health promotion, coaching, and wellness.Ā
PreviousĀ managed care experience.Ā
Bilingual ā English, Spanish.Ā
Certification in Case Management.Ā
Motivational Interviewing Certification and/or knowledge in area.Ā Ā

Workstyle/Travel:Ā This is a HybridĀ āĀ Home position that requires occasional onsite work atĀ theĀ market office in Schaumburg,Ā Illinois, asĀ wellĀ 75-90% travel in the field to visit membersĀ throughout Illinois.

This role is considered patient facing and is part of Humanaās Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance.āÆIndividual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.āÆ
Work at Home Requirements: To ensure Home or Hybrid Home/Office employeesā ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.Ā
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.Ā

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
