The Behavioral Health Care Coach assesses and evaluates member's needs to achieve and maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach Coordinator work assignments are often straightforward and of moderate complexity.
You will report to the Manager, Care Management Behavioral Health.
The Behavioral Health Care Coach employs a variety of strategies, approaches, and techniques to manage a member's health issues. The Behavioral Health Care Coach assesses and evaluates member's needs to establish a member specific care plan and coordinates services.
Identifies and resolves barriers that hinder effective care.
Ensures members are receiving services in the least restrictive setting to achieve and/or maintain optimal well-being by assessing their care needs.
Plan and implement interventions to meet care needs.
Coordinates services, monitors, and evaluates the case management plan against the member's personal goals.
Guides members/families towards resources appropriate for their care. Services are driven by facilitating interactions with other payer sources, providers, interdisciplinary teams and others involved in the member's care as appropriate and required by our comprehensive contract.
You will travel 75-90% of the time to visit Medicaid members as needed in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities and other care settings.
Travel will be in the following counties; Berkeley, Dorchester, Charleston, Colleton, Hampton, Jasper, and Beaufort. You should reside within or just outside these counties.

Required Qualifications
Bachelor's degree in health and human services field
Must be a South Carolina resident
2 or more years of related experience
Exceptional verbal/written communication and interpersonal skills
Use a variety of electronic information applications/software programs
Must have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work
Preferred Qualifications
Prior experience with Medicare & Medicaid recipients
Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems
Experience with health promotion, coaching and wellness
Knowledge of community health and social service agencies and additional community resources
Additional Information
Workstyle: Field will be between 75%-90% of the time to support collaboration, conduct face-to-face meetings, and engage directly with staff, providers, members, and their families.
Location: Travel to Berkeley, Dorchester, Charleston, Colleton, Hampton, Jasper, and Beaufort.
Hours: Must be able to work a 40 hour work week, Monday through Friday 8:00 AM to 5:00 PM, over-time may be requested to meet business needs and requires approval.
Work-At-Home Requirements:
To ensure Home or Hybrid Home/Office associates' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office associates must meet the following criteria:
· At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested.
· Satellite, cellular and microwave connection can be used only if approved by leadership.
· Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
· Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job.
· Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
TB Statement:
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement:
This role is part of our company's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
Medicaid Mileage Reimbursement:
We provide mileage reimbursement for work-related travel. Eligible mileage includes:
Travel from your home to your first work location of the day.
Travel between client or assignment locations during the workday.
Travel from your final work location back to your home.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
