Summary
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties
NOTE: This is a multi-discipline recruitment that will result in the hiring of one of the following disciplines: Nurse Practitioner or Physician Assistant. All Physician Assistant applicants should apply to posting: CBTD-13035419-26-BS. This Nurse Practitioner position is located in the Home Based Primary Care service line at the Tilton/Concord NH location. This position is full-time at 40 hours per week. Included but not limited to: Provide a full range of services emphasizing quality, timeliness, and the advancement of the VA mission. Provides advanced clinical practice, consultation, and management. Practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position. Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Orders and interprets diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals/consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. Requirements
U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
