The Albany VA Healthcare System is hiring for an Advanced Practice Nurse (Nurse Practitioner) to join our Home Based Primary Care / Geriatric and Extend Care Team at the Albany VA Medical Center location. The Advanced Practice Nurse/Certified Nurse Practitioner practices in our Home Based Primary Care Service to provide a full range of services emphasizing quality, timeliness, provides advanced clinical practice, consultation, and management to advance the VA mission.

Duties

The Home-Based Primary Care (HBPC) / Geriatric and Extend Care Nurse Practitioner is accountable for the management and evaluation of a panel of patients under their care in the veteran's home setting or via clinical video telehealth. The HBPC Nurse Practitioner provides comprehensive, preventative, and wellness care, as well as the spectrum of acute, episodic, chronic, and end-of-life care to Veterans. He/she is prepared in advanced practice across the health continuum, and participates in and use of research, development, and implementation of health policy, leadership, education, case management, and consultation. He/she utilizes preventative clinical guidelines and appropriate standards of care in collaboration with the Home-Based Primary Care Medical Director in providing quality care to patients with complex chronic and acute on chronic health conditions. Based on the needs of the Geriatric Service, this position may include coverage in Geriatric outpatient clinics and Community Living Center (long-term stay units) to provide focused interim visits as appropriate. All training and support will be provided for these duties. Major duties and responsibilities include: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develop and implement an individualized plan to achieve optimal population health and disease management. Demonstrate leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Order and interpret labs, tests, and diagnostic studies. Prescribe pharmacologic and non-pharmacologic interventions. Initiate referrals and consults to specialty services. Provide education to patients and their families. Assess and adjusts the plan of care as needed to achieve optimal patient outcomes. Work Schedule: Full-Time. Monday - Friday (8:00 AM - 4:30 PM) - No Holidays/Weekends, No On-call duties. Relocation/Recruitment Incentives: Not Authorized EDRP Authorized: Position(s) listed under this Open Continuous Announcement (OCA) MAY be eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Applicants selected for an EDRP-eligible position will be notified in their conditional job offer. The selectee must meet specific eligibility requirements per VHA policy and submit an EDRP application within four months of appointment. Program approval, award amount (up to $200,000), and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants are ineligible to apply. Requirements

U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Must pass pre-employment physical evaluation. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
