Summary

The Patient Aligned Care Team (PACT) Licensed Practical/License Vocational Nurse (LPN/LVN) 7 performs assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran. Duties

In addition to all areas being recruited for, please note there are two locations for Jacksonville location: Jacksonville North Clinic Jacksonville University Clinic The North Florida/South Georgia Veterans Health System consists of two medical centers, three large multi-specialty outpatient clinics and eight small community-based primary care outpatient clinics. The Malcom Randall VA Medical Center (VAMC) in Gainesville is a tertiary care facility that is also an active teaching hospital, with an extensive array of specialty services. The Malcom Randall VAMC combines a full range of patient care services with state-of-the-art technology that is enhanced and supported through education and research. The LPN may contribute to the plan of care by collaborating with interdisciplinary team members, the patient and the patient's family. The LPN collects data and information, recognizes changes in conditions and reports this to the RN or Provider. The LPN assists in the identification of problems and formulation of goals, outcomes, and patient-centered plans of care that are developed in collaboration with patients, their families, and the interprofessional team. The LPN can participate in implementing established teaching plans and providing education to patients and their family. Duties include but not limited to: Tracks, prepares, and administers medications. Places orders and consults per clinical reminders and PACT Nurse Protocols working to the top of education and competency. Collaborates with PACT staff to develop comprehensive health care plans for patients assigned to patient panels. Participates in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients. Provides effective education to patients and/or family member in relation to disease processes, medication and/or prescribed treatment regimens in collaboration with the interprofessional team. Recognizes urgent or emergent patient care situations, seeks assistance of the RN and/or provider, and initiates appropriate emergency interventions as directed. Performs other duties as assigned. Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
